5. Birthplace: If born before August 15, 1947, in Pakistan or Bangladesh, mention "Undivided Area"

6. Marital status: Your current marital status

7. Gender: Your gender

8. Indian citizenship: Confirmation of your Indian citizenship

9. PAN card and Voter ID card details: Specifications of these identification documents

10. From the available services, select either the re-issuance of a passport (passport renewal) or a new passport. For renewing your passport, you will need to provide a reason from the following options:
