Components of the Ayushman Bharat Scheme

Health and Wellness Centre

The eligibility of the households is based on the deprivation and occupational criteria of the Socio-Economic Caste Census 2011. How to Apply for Ayushman Bharat Yojana
If you are among the vulnerable families identified under the Socio-economic Caste Census, 2011, you are eligible for the scheme. Here is the step-by-step guide to check if you are eligible under the scheme:
Step 1: Visit the website of PMJAY and then click on the “Am I Eligible” button
Step 2: Fill in your contact details and click on “Generate OTP”
Step 3: Select your state and start searching by your name, mobile no., HHD number, etc.
Step 4: After 3rd step, you will be able to check the eligibility
Once you find yourself eligible for the scheme, you must get your Ayushman Bharat Yojana Golden Card. Mentioned below are steps to generate your golden card:
Step 1: Visit the PMJAY website and log in
Step 2: Enter the “Captcha” to generate OTP
Step 3: Opt for the HHD code and provide it to Common Service Centre (CSC) to verify the details.
Step 4: The rest of the process will be completed by Ayushman Mitra, who is the representative of CSC.
Step 5: You need to pay ₹30 to get the golden card after the above process.
